  • Housing Advocate/Mental Health Caseworker
    Housing Advocate/Mental Health Caseworker

    Crawford County Mental Health Awareness Program, Inc. (CHAPS) is seeking a full-time Housing Advocate/Mental Health Caseworker. Duties include providing intensive case management and life skills services to persons who are experiencing homelessness or are near homeless, as well as persons with serious mental illness. This position involves assisting consumers with the identification of and connection to needed community services and resources, including permanent housing.

    Qualifications: Bachelor’s degree in Psychology, Social Work or related human service field or a minimum of 3 years of experience in the human service field. Applicant must possess strong interpersonal and organizational skills. Must have the ability to communicate well, orally and in writing and possess basic computer skills. Ability and willingness to work in consumer’s home, community and with other staff to provide support based on the individual needs of the consumer. Must possess the ability to set priorities and problem solve, as well as complete tasks with minimal supervision. Applicant must be trustworthy, compassionate, and empathetic.

  • CHIPP Residential Support
    CHIPP Residential Support

    CHAPS is seeking recovery-oriented individuals for our CHIPP Residential Support position. This program serves adults experiencing mental illness on an individual basis or in small groups in the home and community to enhance the consumer's quality of life through the promotion of healthy lifestyles, assistance in the achievement of personal goals, and strengthening of independent living skills.
    Duties to include:
    • Assist participants in the development and enhancement of practical living skills needed to maintain independent living
    • Support participants with adherence to medication as prescribed and symptom management
    • Maintain records/documentation and outcomes measures
    • Connect participants to community resources, including both physical and mental health professionals
    • Assisting participants with independent living and social skills
    • Serve as a support system and link to other service providers

    Full and part-time positions are available for caring, dependable workers to provide support to individuals participating in the CHIPP program. The position includes flexible morning, evening, and weekend shifts. Previous experience in human services/mental health preferred, along with strong communication and interpersonal skills and belief that recovery is possible for all.

    We are looking for candidates who have:

    • A high school diploma or GED with previous experience in human services/mental health preferred
    • Current Act 31/33/34 Clearance and FBI Criminal Background Check
    • A valid driver's license

  • Pathfinders Generalist
    Pathfinders Generalist

    We are currently expanding our youth serving programs and we are looking for motivated individuals to join our team. We are offering a part-time position in our youth Site-Based Psychiatric Rehabilitation program, Pathfinders, which meets twice per week after school hours. The Pathfinders program helps bridge the gap between child and adult mental health services for better long-term outcomes for participants. A focus will be on empowering youth to successfully transition from high school, improve overall wellness, and gain skills and competencies for adulthood. Using a Trauma-Informed approach, Pathfinders staff will work with youth participants to complete a strengths-based assessment to help identify areas in their lives in which they would like to develop a goal. These areas, or domains, include living, learning, working, social, and wellness. Youth actively participate in the development of the program including creating community agreements, structuring sessions, and demonstrating voice and choice with activity ideas and skill building projects.
    We are looking for candidates who have:
    • A Bachelor’s degree in a field of study related to social service; OR
    • An Associate’s degree and one year fulltime eligible experience
    • A flexible schedule which includes evenings at least 2x per week
    • Current Act 31/33/34 Clearance and FBI Criminal Background Check
    • A Valid driver's license

  • Host Homes Coordinator
    Host Homes Coordinator

    Crawford County Mental Health Awareness Program, Inc. (CHAPS) is seeking a Host Homes Coordinator. The Host Homes Program is a short-term housing option designed to assist transition age youth experiencing mental health challenges and housing instability in our community. Duties include host recruitment, screening, training, support, and advocacy. This position involves supporting youth/young adults with matching them with a host, developing their Individual Service Plan, connecting them with other support services, and assisting youth with the identification of and connection to needed community services and resources, including permanent housing.

    Qualifications: Bachelor’s degree in psychology, social work or related human service field and 2 years experience in the field of social services, housing, community organizing, or other relevant work. Candidates with relevant experience in place of a BA may apply. Must possess strong interpersonal, communication and organizational skills. Ability and willingness to work in participant’s home, community and with other staff and organizations providing flexible supports based on the individual needs of the participants. Must possess the ability to set priorities and problem solve, as well as complete tasks with minimal supervision. Applicant must be trustworthy, compassionate, and empathetic.
